As a token of sincere appreciation for members’ continuing support, AFPMBAI is pleased to offer the FREE Daily Hospital Income Benefit to members who are hospitalized due to sudden illness, duty-related injuries, or accidents.
Eligibility Criteria
- Regular members in active service with an in-force MBAI Protek or Old Basic Insurance policy
- Retired regular members with an active MBAI iProtek policy
Required Documents*
- Any of the following documents**:
• Narrative Summary
• Clinical Summary
• Medical AbstractThe document must include:
• Description of patient’s day-to-day activities during hospitalization
• Physician’s signature, license number, and contact information
• Dates of admission and discharge, or an indication that the in-patient management is still ongoing
• Date of issuance of the document - Original and photocopy of AFPMBAI ID/Service ID/National ID (with 3 specimen signatures and contact #)
- Latest Payslip (during the month of hospital confinement)
- Duly accomplished DHIB Claim Form including DPA consent
*The above documents obtained after discharge from the hospital shall be required to be submitted by the affected regular member or his/her authorized representative.
**a. If hospital confinement is less than or equal to 10 days, Narrative Summary/Clinical Summary/Medical Abstract may be accepted even without the patient’s day-to-day activities during hospitalization.
b. If hospital confinement is greater than 10 days, Narrative Summary/Clinical Summary/Medical Abstract may only be accepted even without the patient’s day-to-day activities during hospitalization provided that the member-applicant shall be able to submit supporting documents (such as, but not limited to Hospital Statement of Account/Billing Statement, Hospital Records/Details of consultations and treatment, Records/details of operations if surgical operation was performed), or other document/s deemed necessary by the Medical Doctor or Medical Underwriter to further verify the claim.
Additional Requirements (Accident-related hospital confinement)
- Police/NBI Report, Final Spot Report, or Accident Report from immediate Commanding Officer
- Driver’s license if the member-claimant was the driver
Exclusions
- Outpatient care
- Hospital confinement for physical check-up or diagnostic purposes or pregnancy related check-up
- Pregnancy (including childbirth, miscarriage or abortion), and all complications arising therefrom
- Injuries or illnesses which are self-inflicted, caused by suicide attempt or incurred as a result of or while participating in the commission of a crime
- Rest cures, custodial domiciliary, or convalescent cares
- Cosmetic surgery, and surgery for purpose of beautification
- Functional disorders of the mind in relation to substance abuse, drug addiction, or alcoholism
- Injury sustained while under the influence of alcohol or drugs
- Sexually transmitted diseases
- Provoked assault
- Atomic explosion, nuclear fission, or radioactive gas
