CUSTOMER SATISFACTION SURVEY RAFFLE PROMO
Promotion Mechanics
I. GEOGRAPHICAL SCOPE AND DURATION
This NATIONWIDE promo will run from MAY 1, 2024 TO DECEMBER 31, 2024.
II. MECHANICS
The promo is open to ALL REGULAR UNIFORMED SERVICES MEMBERS and ASSOCIATE MEMBERS of the AFPMBAI, as defined in attached Definition of Terms (Attachment A).
1. Participants must be Filipino citizens, at least 18 years of age, and residing in the Philippines.
2. AFPMBAI members will be invited to participate in the Customer Satisfaction Survey through a web link (https://bit.ly/AFPMBAICSatSurvey) and/or QR code. Each member can answer the survey only once, which will entitle the member to one raffle entry.
3. There will be 5 winners from each region (NCR, NOLA, SOLA, VIS, MIN) with a total of 25 winners each month. Each winner will receive PhP 1,000. Over the course of the promotion, there will be a total of 200 winners.
4. Valid entries will be consolidated at the end of each month. The monthly raffle will be conducted offline on the 10th working day following the end of each month.

5. Winners can only win once during the entire promotion. Non-winning entries will be added to the next draw date.
6. Winners will be subject to verification and will be announced on social media after the verification of winners is complete.
7. By participating in this promo, all winners and participants grant AFPMBAI permission to use and disclose personal information and photographs in connection with the promotion for marketing purposes and waive any claims to royalty, right, or remuneration for such use on TV, radio, website, or official social media pages.
III. DRAWING OF WINNERS
1. Each valid entry will be assigned a corresponding raffle number for the raffle draw.
2. The winners will be drawn by region in the following order: NCR, NOLA, SOLA, VIS, MIN. A member’s name can only be drawn once.
3. The raffle draw will be conducted offline as scheduled under Section II, Para 4 of this document. Drawing of winners will be documented.
IV. ANNOUNCEMENT OF WINNERS
Winners will be notified via SMS, e-mail, and/or call out of a representative from the Marketing Division.
V. AWARDING OF WINNERS
1. Prizes must be claimed within thirty (30) calendar days from receipt of notification from AFPMBAI. Otherwise, the prizes shall be forfeited and retained by AFPMBAI.
2. Upon notification of winning, verified winners will be required to answer a form to provide their bank account details, picture of ATM card, valid government or service ID, and picture with their ID as proof of identity.
VI. CLAIMING OF PRIZES
Cash prizes will be deposited to the bank account as provided by the member-winner.
ATTACHMENT A – DEFINITION OF TERMS
a. Regular Members – The following are considered regular members:
● Military personnel in active service of the Armed Forces of the Philippines (AFP)
● Those in active service of the following:

b. Associate Members – The following are considered associate members
● Members of the Reserve Officer Training Corps (ROTC)
● Reservists of the AFP
● Members of the Citizen Armed Forces Geographical Unit Active Auxiliary (CAFGUAA), and Special Civilian Armed Auxiliary (SCAA), Volunteer Fire Brigade, Bureau of Corrections, and other uniformed service units.
● Personnel from the Bureau of Corrections, Philippine Drug Enforcement Agency, National Bureau of Investigation, Bureau of Immigration, National Mapping Resource Information Authority, Airport Police, and Customs Police
● Parents, dependents, wives and children of regular members
● Duly licensed private security guards
● Civilian employees of the AFPMBAI, other AFP financial institutions, non-uniformed personnel of the PNP, and other organizations and agencies connected with the uniformed services and major services bureaus
● Graduates of the Philippine Merchant Marine Academy (PMMA), Philippine National Police Academy (PNPA), and Maritime Academy of Asia and the Pacific (MAAP)
