Scope of Work / Services
- Supply and delivery of Nutanix storage equipment and other required materials.
- Installation and integration of the Nutanix storage to the existing Nutanix server.
- Testing and fine-tuning to ensure proper integration into the existing infrastructure.
Project Duration / Delivery Schedule
- Delivery of equipment: within 60 calendar days from Notice to Proceed.
- Implementation: 3 days after delivery.
- Fine-tuning: 1 day after implementation.
- Maximum allowable extension: up to 120 days, subject to AFPMBAI approval.
Mode of Payment
- 50% upon contract signing with the selected provider (mobilization/down payment).
- 50% upon completion and acceptance of deliverables.
Bid Submission Requirements (Sealed Envelopes)
- Envelope A: Company Profile (Eligibility).
- Envelope B: Commercial Proposal + Project Implementation Proposal.
- Envelope C: Draft Services Agreement.
Bidding Documents Cost and Release
The cost of Bidding Documents shall be One Thousand Pesos (Php 1,000.00). Bidders shall pay the stated amount directly to the AFPMBAI Cashier. For this purpose, bidders shall secure the required payment form from the BAC Secretariat. The payment made shall be non-refundable.
The Bidder shall submit a Letter of Intent and attachments either thru email: jgr.villanueva@afpmbai.com.ph / Viber No. 09173170966 – BAC Secretariat or submit the hard copy to the following address: BAC Services Secretariat, AFPMBAI Bldg., Col Bonny Serrano Road cor EDSA, Quezon City.
